Auto
Insurance
In cases involving auto collisions, we will file the insurance claim
for you so that the payment can be made directly to this office.
Often, any co-payments and deductibles are covered by the other
driver's insurance or your health insurance. Ultimately, you are
responsible for any uncovered portion.
Workers's
Compensation
Once we have received written authorization from your employer/carrier,
you can be evaluated and/or treated.
Health
Insurance Assignment (PPO, HMO, etc.)
With this option, we will file the insurance claim for you and agree
to wait for the payment from the insurance company. You are responsible
for all co-payments, deductibles, and charges for non-covered services.
Time
of Service Discount
This
discount option is intended for you, if you:
1)
Do not have health insurance.
2)
Have insurance (including Medicare) but the chiropractic coverage
is not adequate to treat your health needs.
3)
Have an insurance plan that does not include Dr. Selley as a provider.
4)
Have insurance and wish to file your own claim.
The
TIME OF SERVICE DISCOUNT option has two requirements:
1)
Payment in full must be made at the time of service, and
2)
Our office does not file any insurance claim.

Medicare
Participating Provider
Dr.
Selley is a Medicare participating provider (Medicare assignment
accepted).
"Medicare
assignment accepted" means that the doctor will file the Medicare
claims and wait for payment for covered services from Medicare.
Under Medicare Law, the patient is still responsible for any deductible
and/or co-payment amounts.
Chiropractic
Coverage in Medicare
Medicare
provides coverage for chiropractic adjustments (spinal manipulation)
only. Medicare will not reimburse for examinations, x-rays, physical
therapy, or other services performed or ordered by a chiropractic
physician.
Your
deductible and co-payments for covered services (spinal manipulation)
should be paid by your Medicare supplemental policy. Your Medicare
supplement will not likely apply to non-covered services.
If
you belong to a Medicare HMO or other managed care plan that does
not include Dr. Selley, please contact your insurance company and
request that Dr. Selley be added to the panal of physicians.
Non-Covered Services
Chiropractic
services such as examinations, x-rays, and physical therapy (ultrasound,
electrical muscle stimulation, etc.) are not covered under regular
Medicare. As a result, you can choose the TIME OF SERVICE DISCOUNT
and receive a substantial savings for these services. This is a
popular choice for patients who desire to benefit from electrical
muscle stimulation and ultrasound.
Treatment Choices
Spinal
adjustments (spinal manipulations) are the primary treatment modality
performed in our chiropractic office. We also have state-of-the-art
physical therapy equipment to provide electrical muscle stimulation
and ultrasound. These physical therapy modalities are often helpful
to speed the recovery process and enhance the effects of the spinal
adjustments. Even though electrical muscle stimulation and ultrasound
are not covered by Medicare in a chiropractic office, many patients
choose to benefit from these treatment modalities by taking advantage
of the TIME OF SERVICE SERVICE DISCOUNT.
When
Dr. Selley discusses your treatment options with you, he may recommend
only spinal adjustments or a combination of spinal adjustments and
physical therapy. Dr. Selley will work with you to tailor a treatment
approach designed to resolve you health problems while taking into
account any Medicare or financial concerns you may have. The decision
is ultimately yours as to which treatment path you wish to folllow.
